Attention Parents: SB 12 – Medical Consent Forms Sent Home Today

Dear Goliad ISD Families,
In accordance with Texas Senate Bill 12, school nurses and campus staff may only provide medical treatment in a true emergency unless we have a signed medical consent form on file for your child.
Hard copies of this form were sent home with students today.
Please complete and return the form to your child’s campus as soon as possible so that we can provide care if needed.
If you choose “No” on the form, please be aware that we will be unable to provide care for your child in non-emergency situations, and you will be contacted to pick them up if they report feeling unwell at school.
If you have questions or need another copy of the form, please contact your campus nurse or front office.
